New Patient Registration
You may register with the practice if you live within our practice boundary. You will be asked to produce your medical card or alternatively, to fill in a form for each member of the family.
If you are a newly registered patient requesting repeat medication, you will first need an appointment with a doctor or the practice pharmacist.
Medical treatment is available from the date of registration. Please contact reception for further information.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
It would be helpful if you bring in your NHS number and ID containing your full name and date of birth however this is not compulsory.

Named Accountable GP
When registering at Lombard Medical Centre you will be allocated a named GP who will be responsible for your overall care while you at the practice. You can however request an appointment with a GP of your choice.
If you wish to find out who your named GP is then please ask at reception.
